Forms & Strengths
- Intravenous injection: Colloidal suspension of superparamagnetic iron oxide nanoparticles (SPIO)
Adult Dosing
- Diagnostic imaging (MRI): Typically administered as an IV bolus or slow infusion per specific agent protocol (e.g., 0.56 mg iron/kg for liver/spleen imaging)
Pediatric Dosing
- Safety and efficacy not well established in pediatric patients; use is generally off-label and restricted to specialized diagnostic centers
Indications
- Magnetic resonance imaging (MRI) contrast agent for enhancement of the liver, spleen, and lymph nodes
Mechanism of Action
- Superparamagnetic iron oxide nanoparticles are taken up by reticuloendothelial system (RES) macrophages in healthy liver and spleen tissue, shortening T2 and T2* relaxation times and creating signal voids on MRI
Contraindications
- Known hypersensitivity to iron oxide nanoparticles or parenteral iron products
- Hemochromatosis or other iron overload syndromes
Adverse Reactions
- Transient back or lower lumbar pain
- Hypotension
- Flush and warmth
- Allergic or anaphylactoid reactions
- Nausea and vomiting
Drug Interactions
- Concomitant use with other parenteral iron formulations may increase the risk of acute iron toxicity
- May cause artifacts on subsequent MRI scans if significant iron deposition persists
