technetium (99mTc) exametazime
V09AA01
Forms & Strengths
- Lyophilized kit (10 mg) for reconstitution with 99mTc pertechnetate
- Single‑use vial, 30 mg kit
- Reconstituted solution up to 740 MBq (20 mCi) per dose
Adult Dosing
- IV injection 370 MBq (10 mCi) for cerebral perfusion SPECT
- Dose range 370‑740 MBq (10‑20 mCi) depending on imaging protocol
Pediatric Dosing
- Weight‑based 5‑10 MBq/kg IV (max 370 MBq)
- For children >5 kg, typical dose 185‑370 MBq
Indications
- Brain perfusion SPECT for stroke evaluation
- Localization of epileptic focus
- Assessment of dementia and neurodegenerative disease
- Brain death determination
- Evaluation of cerebral blood flow abnormalities
Mechanism of Action
- Lipophilic Tc‑99m exametazime crosses the blood‑brain barrier
- Localization proportional to regional cerebral blood flow
- Emits 140 keV gamma photons detected by SPECT
Contraindications
- Known hypersensitivity to exametazime or kit excipients
- Pregnancy (relative contraindication)
- Breastfeeding (caution)
Adverse Reactions
- Mild injection site pain or erythema
- Transient nausea or flushing
- Rare allergic reaction (urticaria, anaphylaxis)
- Hypotension (very rare)
Drug Interactions
- Agents altering cerebral blood flow (e.g., vasodilators, caffeine, nicotine) may affect uptake
- High‑dose steroids may reduce brain uptake
- Concurrent radiopharmaceuticals within 24 h may cause interference
