technetium (99mTc) bicisate
V09AA02
Forms & Strengths
- Kit for the preparation of technetium Tc 99m bicisate injection
- Lyophilized powder in a vial containing 0.9 mg bicisate dihydrochloride
Adult Dosing
- 740 to 1110 MBq (20 to 30 mCi) administered as a single intravenous injection
- Imaging typically performed 15 to 90 minutes post-injection
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
- Use in children is generally avoided unless clearly indicated and calculated based on body weight
Indications
- Localization of regional cerebral blood flow in evaluation of stroke
- Assisting in the localization of seizure foci in patients with epilepsy
Mechanism of Action
- Lipophilic complex that crosses the blood-brain barrier intact
- Converted into more polar, hydrophilic metabolites by enzymatic hydrolysis within brain cells
- Trapped intracellularly in proportion to regional cerebral blood flow
Contraindications
- Hypersensitivity to technetium Tc 99m bicisate or any component of the formulation
- Pregnancy, unless the potential benefit outweighs the unknown risks of radiation exposure
Adverse Reactions
- Generally well-tolerated with rare adverse events
- Headache, dizziness, and nausea reported infrequently
- Transient injection site pain or erythema
Drug Interactions
- Drugs that alter regional cerebral blood flow (e.g., vasodilators, vasoconstrictors, CNS active agents) may alter radiotracer distribution
- No specific pharmacokinetic drug-drug interactions documented
