technetium (99mTc) pentetic acid
V09CA01
Forms & Strengths
- Kit for the preparation of technetium Tc 99m pentetate (DTPA) injection
- Lyophilized powder containing pentetate calcium trisodium and stannous chloride
Adult Dosing
- Brain imaging: 10-20 mCi (370-740 MBq) intravenously
- Renal function/perfusion imaging: 3-5 mCi (111-185 MBq) intravenously
- Vesicoureteral reflux (cystography): 0.5-3 mCi (18.5-111 MBq) intravesically
- Lung ventilation (aerosol): 25-35 mCi in nebulizer; patient inhales 1-2 mCi (37-74 MBq)
Pediatric Dosing
- Renal imaging: 0.05-0.15 mCi/kg (minimum 0.5 mCi) intravenously
- Vesicoureteral reflux: 0.5-1 mCi intravesically
- Lung ventilation: 0.5-1 mCi delivered dose via nebulizer
Indications
- Evaluation of renal perfusion and function (glomerular filtration)
- Radionuclide cisternography for cerebrospinal fluid leaks and shunt patency
- Brain scintigraphy to assess intracranial lesions and blood-brain barrier integrity
- Inhalation lung scintigraphy for evaluation of pulmonary ventilation
Mechanism of Action
- Filtered freely by the renal glomeruli without tubular secretion or reabsorption for renal imaging
- Crosses the blood-brain barrier only when integrity is disrupted
- Inhaled aerosol deposits in the central and peripheral airways for ventilation studies
- Gamma emission (140 keV) allows external detection via gamma camera
Contraindications
- Hypersensitivity to pentetate sodium/calcium or any components of the kit
- Pregnancy, unless the clinical benefit outweighs the potential risk of radiation
Adverse Reactions
- Hypersensitivity and anaphylactoid reactions (rare: urticaria, pruritus, hypotension)
- Injection site reactions
- Radiation exposure associated with diagnostic imaging
Drug Interactions
- Drugs altering renal hemodynamics (e.g., ACE inhibitors, diuretics) may affect renal scintigraphy results
- Certain chemotherapeutic agents or nephrotoxic drugs can alter renal clearance kinetics
