technetium (99mTc) succimer
V09CA02
Forms & Strengths
- Lyophilized Tc-99m DMSA kit (2.5 mg DMSA per vial)
- Reconstituted solution 1–2 mCi/mL
- Available in 5‑10 mCi (185‑370 MBq) dose vials
Adult Dosing
- 5 mCi (185 MBq) IV for routine renal cortical imaging
- 10 mCi (370 MBq) IV for detailed cortical defect assessment
- Imaging performed 2–3 h post‑injection
Pediatric Dosing
- 2 mCi (74 MBq) IV for children ≤5 kg
- 5 mCi (185 MBq) IV for children 5–25 kg
- Dose generally 0.05 mCi/kg (1.85 MBq/kg) up to 10 mCi
Indications
- Renal cortical scintigraphy to assess cortical anatomy
- Detection of renal cortical scarring after pyelonephritis
- Evaluation of renal transplant function
- Localization of neuroblastoma and other DMSA‑avid tumors
Mechanism of Action
- Tc‑99m bound to dimercaptosuccinic acid localizes in proximal tubular cells via megalin‑cubilin mediated endocytosis
- Decay of Tc‑99m emits 140 keV photons captured by gamma camera
Contraindications
- Known hypersensitivity to DMSA or kit excipients
- Pregnancy – use only if benefit outweighs risk
Adverse Reactions
- Mild nausea or vomiting
- Urticaria and pruritus
- Rare anaphylactic reaction
- Transient rise in serum creatinine
Drug Interactions
- Concurrent chelating agents (e.g., dimercaprol) may reduce renal uptake
- No clinically significant interactions with most drugs
- Avoid simultaneous administration of other renal‑targeted radiopharmaceuticals
