Forms & Strengths
- Sterile aqueous solution for IV injection, 123I‑iodohippurate (sodium iodohippurate)
- Typical supplied activity 0.5–5 GBq per vial depending on manufacturer
Adult Dosing
- IV bolus 3–5 MBq (0.08–0.14 mCi) for planar renal cortical imaging; up to 10 MBq for SPECT/CT
- Inject rapidly, followed by 10 mL saline flush
Pediatric Dosing
- Weight‑based 0.5–1 MBq/kg IV (maximum 5 MBq for infants)
- Adjust activity for age‑related renal function; use lowest activity that yields diagnostic quality
Indications
- Renal cortical scintigraphy to assess differential renal function
- Detection of renal scarring or cortical defects (e.g., reflux nephropathy)
- Evaluation of renal tubular function and effective renal plasma flow
Mechanism of Action
- 123I‑iodohippurate is actively taken up by proximal tubular cells via organic anion transporters
- Retention reflects renal cortical perfusion and tubular function; minimal glomerular filtration
Contraindications
- Known hypersensitivity to iodinated compounds or sodium iodohippurate
- Pregnancy unless benefit outweighs radiation risk
- Breast‑feeding – discontinue or pump and discard milk for 24 h
Adverse Reactions
- Mild nausea, vomiting, or flushing during injection
- Transient rash or urticaria; rare anaphylactoid reaction
- Radiation exposure – very low but contraindicated in pregnant women
Drug Interactions
- Drugs inhibiting tubular secretion (e.g., probenecid, cimetidine, diuretics) may reduce renal uptake
- High‑dose iodine or iodinated contrast may compete for transport and lower image quality
