sodium iodohippurate (131I)
V09CX02
Forms & Strengths
- Injection solution, diagnostic radiopharmaceutical
- Typically supplied as 37 MBg (1 mCi) per vial at calibration time
Adult Dosing
- Intravenous injection: 0.185 to 3.7 MBg (5 to 100 microcuries) for renal function studies
- Dose adjusted based on specific diagnostic procedure (e.g., effective renal plasma flow or renography)
Pediatric Dosing
- Intravenous injection: Dose calculated on a per-kilogram or body surface area basis, typically ranging from 0.05 to 0.15 MBg/kg
- Minimum administered activity determined by gamma camera sensitivity and clinical protocol
Indications
- Measurement of effective renal plasma flow (ERPF)
- Radionuclide renography and assessment of individual kidney function
- Evaluation of renal transplant patency and urinary tract obstruction
Mechanism of Action
- Radiolabeled hippurate analogue transported rapidly across the renal tubular epithelium via organic anion transporters
- Eliminated almost exclusively by tubular secretion with minimal glomerular filtration, allowing measurement of effective renal plasma flow
Contraindications
- Hypersensitivity to iodinated compounds or any component of the formulation
- Pregnancy, due to potential risks of ionizing radiation to the fetus
- Breastfeeding (temporary discontinuation required)
Adverse Reactions
- Hypersensitivity reactions (urticaria, rash, pruritus, anaphylaxis)
- Nausea and flushing rarely reported
- Injection site reactions
Drug Interactions
- Probenecid inhibits renal tubular secretion, significantly altering effective renal plasma flow measurements
- Drugs altering renal hemodynamics (e.g., NSAIDs, ACE inhibitors) may affect diagnostic test interpretation
