Forms & Strengths
- Lyophilized kit for reconstitution with 99mTc pertechnetate
- Radiopharmaceutical solution ready for IV injection
- Typical activity 185‑370 MBq (5‑10 mCi) per adult dose
Adult Dosing
- IV bolus 185‑370 MBq (5‑10 mCi) for renal cortical imaging
- Higher dose (up to 740 MBq) may be used for delayed imaging or low‑weight adults
Pediatric Dosing
- Weight‑based IV dose 1.85‑3.7 MBq/kg (0.05‑0.1 mCi/kg), max 185 MBq (5 mCi)
- Infants <5 kg: 74‑111 MBq (2‑3 mCi)
Indications
- Renal cortical scintigraphy for detection of obstruction, scarring, and differential function
- Evaluation of renal tubular function and drainage studies
- Assessment of urinary tract anomalies in children
Mechanism of Action
- Disofenin peptide binds to megalin‑cubilin receptors in proximal tubules, causing rapid tubular uptake and retention
- 99mTc emits 140 keV gamma photons for planar or SPECT imaging
Contraindications
- Known hypersensitivity to disofenin, kit excipients, or any component of the radiopharmaceutical
- Pregnancy unless the diagnostic benefit outweighs fetal radiation risk
- Breastfeeding – discontinue nursing for 24 h after administration
Adverse Reactions
- Mild nausea or vomiting (transient)
- Urticaria or pruritus
- Rare anaphylactoid reaction or bronchospasm
- Transient rise in serum creatinine in patients with severe renal impairment
Drug Interactions
- Loop diuretics and osmotic diuretics may reduce tubular uptake and image quality
- ACE inhibitors, ARBs, or NSAIDs can alter renal perfusion and affect tracer distribution
- Concurrent administration of other radiopharmaceuticals may cause cross‑contamination of gamma counts
- Iodinated contrast agents administered within 24 h may interfere with renal imaging
