Forms & Strengths
- Kit for the preparation of technetium Tc 99m lidofenin injection (lyophilized powder)
- Radiopharmaceutical diagnostic agent requiring reconstitution with sodium pertechnetate Tc 99m
Adult Dosing
- Intravenous injection of 37 to 185 MBq (1 to 5 mCi) of technetium Tc 99m lidofenin
- Administered as a single diagnostic dose prior to hepatobiliary scintigraphy imaging
Pediatric Dosing
- 0.05 to 0.1 mL/kg of reconstituted solution administered intravenously
- Minimum recommended dose is 0.5 mCi (18.5 MBq) to ensure adequate visualization
- Safety and efficacy in neonates and infants under strict clinical evaluation
Indications
- Hepatobiliary imaging for differential diagnosis of acute cholecystitis
- Evaluation of biliary tract patency, bile leaks, and congenital biliary abnormalities such as biliary atresia
Mechanism of Action
- Lidofenin is selectively taken up by hepatocytes via active transport mechanisms in the liver
- Rapidly excreted into the bile, outlining the biliary tree, gallbladder, and small intestine
- Radiolabeled with technetium-99m to allow gamma camera scintigraphy visualization of hepatobiliary function
Contraindications
- Known hypersensitivity to lidofenin or other iminodiacetic acid (IDA) derivatives
- Pregnancy and lactation unless diagnostic benefits clearly outweigh potential radiation risks
Adverse Reactions
- Rare hypersensitivity reactions including rash, urticaria, and pruritus
- Transient nausea, dizziness, or flushing during intravenous administration
Drug Interactions
- Opioid analgesics may cause spasm of the sphincter of Oddi, altering hepatobiliary transit times and mimicking biliary obstruction
- Recent meals or total parenteral nutrition stimulate gallbladder contraction or keep it contracted, interfering with optimal diagnostic visualization
