technetium (99mTc) nanocolloid
V09DB01
Forms & Strengths
- Kit for the preparation of technetium Tc 99m nanocolloid injection: multi-dose vial containing human albumin nanocolloid, stannous chloride dihydrate, glucose, and poloxamer 188.
Adult Dosing
- Lymphoscintigraphy and sentinel lymph node localization: 20 to 120 MBg (0.5 to 3.2 mCi) administered via intradermal, subcutaneous, submucosal, or peritumoral injection, divided into multiple sites depending on the tumor location.
- Imaging is typically performed from 10 minutes to several hours post-injection.
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established; use is generally restricted to specialized pediatric nuclear medicine centers with activity adjusted based on body weight or surface area according to EANM/SNMMI pediatric dosage card.
Indications
- Sentinel lymph node detection in melanoma, breast cancer, and other solid tumors requiring lymphatic mapping.
- Lymphoscintigraphy for the assessment of the lymphatic system and lymphatic drainage disorders.
Mechanism of Action
- Technetium Tc 99m nanocolloid consists of human serum albumin nanoparticles (mostly between 20 and 80 nm in diameter) that are radiolabeled with technetium-99m.
- Following interstitial injection, the nanoparticles enter the lymphatic capillaries due to their small size, are transported via lymph flow, and are sequentially trapped and phagocytosed by macrophages in the sentinel lymph nodes, allowing gamma camera or SPECT/CT visualization.
Contraindications
- Hypersensitivity to the active substance, human serum albumin, or any excipients in the kit.
- General contraindications associated with radioactive diagnostic agents and pregnancy (unless the clinical benefit outweighs the risk).
Adverse Reactions
- Hypersensitivity and anaphylactoid reactions (rare, including rash, urticaria, pruritus, dyspnea, and hypotension due to human albumin content).
- Pain, erythema, or transient inflammation at the injection site.
Drug Interactions
- No formal drug-interaction studies have been performed.
- Prior radiotherapy, surgery, or large-tumor burden may alter lymphatic drainage patterns, leading to false-negative sentinel node identification or non-visualization.
