Forms & Strengths
- Kit for the preparation of technetium Tc 99m pentetate (DTPA) injection
- Liofilized powder containing pentetate calcium trisodium and stannous chloride
Adult Dosing
- Brain imaging: 370 to 740 MBq (10 to 20 mCi) intravenous
- Renal imaging (perfusion/functional): 185 to 370 MBq (5 to 10 mCi) intravenous
- Vesicoureteral reflux (cystography): 18.5 to 37 MBq (0.5 to 1.0 mCi) intravesical administration
- Lung ventilation (aerosol): 925 to 1850 MBq (25 to 50 mCi) in nebulizer; patient inhales 18.5 to 37 MBq (0.5 to 1.0 mCi)
Pediatric Dosing
- Brain imaging: 7.4 to 18.5 MBq/kg (0.2 to 0.5 mCi/kg) intravenous, minimum 18.5 MBq (0.5 mCi)
- Renal imaging: 3.7 to 7.4 MBq/kg (0.1 to 0.2 mCi/kg) intravenous, minimum 18.5 MBq (0.5 mCi)
- Vesicoureteral reflux: 18.5 to 37 MBq (0.5 to 1.0 mCi) intravesical administration
Indications
- Evaluation of renal function, perfusion, and urinary tract obstruction
- Assessment of cerebroventricular shunt patency and brain imaging for lesions
- Lung ventilation imaging for pulmonary embolism evaluation when nebulized
- Evaluation of vesicoureteral reflux via direct radionuclide cystography
Mechanism of Action
- Chelated radioactive technetium-99m emits gamma radiation for scintigraphic imaging
- Renal elimination occurs primarily via glomerular filtration without tubular secretion or reabsorption
- Aerosolized form deposits in the central and peripheral airways via inhalation for ventilation scans
Contraindications
- Hypersensitivity to technetium Tc 99m pentetate or any component of the kit
- Pregnancy, unless the clinical benefits outweigh the potential risks of radiation exposure
- Lactation, temporary discontinuation of breastfeeding is recommended
Adverse Reactions
- Hypersensitivity reactions including rash, urticaria, pruritus, and angioedema
- Anaphylactic reactions rarely reported
- Injection site reactions such as pain or erythema
- Radiation-induced risks associated with all radiopharmaceuticals
Drug Interactions
- Drugs altering renal hemodynamics (e.g., ACE inhibitors, diuretics) may affect renal scintigraphy interpretation
- Chemotherapeutic agents or nephrotoxic drugs can alter renal handling and biodistribution
- No formal pharmacokinetic drug interaction studies have been conducted
