Forms & Strengths
- Kit for the preparation of technetium Tc 99m macrosalb injection: lyophilized powder in multiple-dose vials
Adult Dosing
- Lung scintigraphy: 37 to 148 MBq (1 to 4 mCi) administered intravenously
- Dose determined by number of particles (typically 200,000 to 700,000) to avoid excessive occlusion of pulmonary capillaries
Pediatric Dosing
- Lung scintigraphy: 0.5 to 1.5 MBq/kg body weight administered intravenously
- Minimum recommended dose: 7.4 MBq (0.2 mCi)
- Particle number reduced proportionally based on age and body surface area
Indications
- Lung scintigraphy for the assessment of pulmonary perfusion
- Evaluation of suspected pulmonary embolism
- Right-to-left shunt detection (with brain or whole-body imaging)
Mechanism of Action
- Technetium Tc 99m macroaggregated albumin (MAA) particles are trapped temporarily in the pulmonary capillary bed via microembolization
- Distribution of trapped radioactive particles is proportional to regional pulmonary blood flow
Contraindications
- Known hypersensitivity to human serum albumin or technetium Tc 99m preparations
- Severe pulmonary hypertension (relative contraindication due to risk of complete capillary bed blockage)
- Right-to-left cardiac shunts require significant dose reduction to prevent cerebral or systemic microembolization
Adverse Reactions
- Hypersensitivity and anaphylactic reactions (rare)
- Fever, chills, nausea, and flushing (very rare)
- Transient dyspnea or chest discomfort in patients with severe pulmonary hypertension
Drug Interactions
- Substances that alter regional pulmonary blood flow can affect scan interpretation
- Previous administration of other particulate radiopharmaceuticals may interfere with particle number calculations
