technetium (99mTc) pertechnetate
V09FX01
Forms & Strengths
- Sodium pertechnetate solution for injection, 0.5‑5 mL vials, up to 3 GBq per vial
- Ready‑to‑use sterile isotonic kit
- Gamma emitter (140 keV) used for scintigraphy
Adult Dosing
- Thyroid scan: 370‑740 MBq (10‑20 mCi) IV
- Salivary gland scan: 185‑370 MBq (5‑10 mCi) IV
- Gastric mucosa scan: 185‑370 MBq (5‑10 mCi) IV
- Renal cortical scan: 370 MBq (10 mCi) IV
- Brain perfusion SPECT: 740 MBq (20 mCi) IV
- Meckel’s diverticulum scan: 185‑370 MBq (5‑10 mCi) IV
Pediatric Dosing
- 0.05‑0.1 mCi/kg (1.85‑3.7 MBq/kg) IV, max 370 MBq (10 mCi)
- Thyroid scan: 0.05 mCi/kg IV
- Salivary gland scan: 0.05‑0.07 mCi/kg IV
- Gastric scan: 0.05 mCi/kg IV
- Renal scan: 0.07 mCi/kg IV
Indications
- Thyroid uptake and scintigraphy (hyper‑/hypothyroidism, nodules)
- Salivary gland (parotid, submandibular) imaging
- Gastric mucosal perfusion and emptying studies
- Renal cortical scintigraphy for differential function
- Brain perfusion SPECT (stroke, epilepsy)
- Meckel’s diverticulum detection (ectopic gastric mucosa)
- Pericardial blood‑pool imaging
Mechanism of Action
- Emits 140 keV gamma photons detected by gamma camera
- Uptake via Na+/I‑ symporter in thyroid, gastric mucosa, salivary glands
- Passive diffusion into extracellular fluid reflects regional blood flow
- No metabolic transformation; clearance renally
Contraindications
- Pregnancy (relative contraindication)
- Breastfeeding (avoid unless benefit outweighs risk)
- Known hypersensitivity to pertechnetate or kit excipients
- Inability to lie still for imaging acquisition
Adverse Reactions
- Rare hypersensitivity reactions (urticaria, pruritus)
- Nausea or vomiting
- Transient flushing or hot flashes
- Injection‑site pain or erythema
- Radiation‑induced stochastic risk (theoretical, low)
Drug Interactions
- Iodine‑containing compounds (contrast media, KI, amiodarone) reduce thyroid uptake
- Thyroid hormone therapy may alter thyroid scan appearance
- Lithium increases thyroid uptake
- Diuretics (e.g., furosemide) enhance renal excretion and improve renal images
- Anticholinergics decrease salivary gland uptake
