Forms & Strengths
- Radiopharmaceutical diagnostic agent
- Kit for the radiolabeling of autologous red blood cells or leukocytes with Chromium-51
Adult Dosing
- Red cell volume/survival: 10 to 50 microcuries (0.37 to 1.85 MBq) IV of labeled red blood cells
- Spleen sequestration/gastrointestinal protein loss: 15 to 30 microcuries (0.55 to 1.11 MBq) IV
- Leukocyte labeling: 10 to 20 microcuries (0.37 to 0.74 MBq) IV for localization of inflammation/infection
Pediatric Dosing
- Safety and efficacy not well established in pediatric patients
- Dosing must be reduced based on body weight or surface area according to nuclear medicine guidelines
Indications
- Measurement of red blood cell volume and mass
- Determination of red blood cell survival time
- Study of gastrointestinal protein loss
- Localization of sites of inflammation or infection using radiolabeled leukocytes
Mechanism of Action
- Chromium-51 binds firmly to the beta chain of hemoglobin within erythrocytes or to intracellular components of leukocytes
- Emits gamma radiation (primarily 0.32 MeV) which allows external detection and quantification via scintillation counting for survival, volume, and biodistribution studies
Contraindications
- Known hypersensitivity to chromium or any components of the radiolabeling procedure
- Pregnancy, unless the diagnostic benefit outweighs the significant radiation risks to the fetus
- Lactation (breastfeeding should be interrupted following administration)
Adverse Reactions
- Hypersensitivity reactions including rash, urticaria, and anaphylaxis
- Radiation exposure associated with potential long-term stochastic risks
- Extravasation at injection site may cause local tissue irritation
Drug Interactions
- Any agent or condition causing active hemolysis can falsely shorten red blood cell survival times
- Recent blood transfusions invalidate red blood cell volume and survival measurements
- Anticoagulants and cytotoxic drugs may affect leukocyte viability and labeling efficiency
