technetium (99mTc) human immunoglobulin
V09HA01
Forms & Strengths
- Lyophilized kits for reconstitution with 99mTc pertechnetate
- Standard activity: 10-30 mCi (370-1110 MBq) per dose
- Available in single-use vials
Adult Dosing
- 0.1-0.2 mCi/kg (3.7-7.4 MBq/kg) IV injection
- Usual adult total activity 5-10 mCi (185-370 MBq) for most studies
- Maximum single dose 30 mCi (1110 MBq) to limit radiation exposure
Pediatric Dosing
- 0.1-0.2 mCi/kg IV, same as adult per kg
- Do not exceed 5 mCi (185 MBq) total activity in children <12 kg
- Adjust dose for age/weight to keep effective radiation dose <5 mSv
Indications
- Localization of infection and inflammation (e.g., osteomyelitis, septic arthritis)
- Detection of soft‑tissue abscesses and cellulitis
- Evaluation of prosthetic joint infection
- Assessment of fever of unknown origin when infection is suspected
- Imaging of inflammatory bowel disease activity
Mechanism of Action
- Human IgG binds Fc receptors on activated leukocytes at sites of infection/inflammation
- 99mTc provides gamma emission (140 keV) for planar or SPECT imaging
- Accumulation reflects increased vascular permeability and leukocyte migration
Contraindications
- Known hypersensitivity to human immunoglobulin or any kit excipient
- Severe IgA deficiency with anti‑IgA antibodies
- Pregnancy: avoid unless benefit outweighs fetal radiation risk
- Breastfeeding: discontinue nursing for 24 h post‑administration
Adverse Reactions
- Mild: flushing, warmth, nausea, headache
- Urticaria or pruritus at injection site
- Rare: anaphylactoid reaction or severe hypersensitivity
- Radiation‑related: transient marrow suppression at high cumulative doses
Drug Interactions
- Concurrent administration of other radiopharmaceuticals may increase total radiation dose
- High serum iron or reducing agents can reduce labeling efficiency
- Immunosuppressants may diminish target uptake, affecting image quality
- Heparin therapy does not interfere but monitor for bleeding if large volumes are infused
