technetium (99mTc) pentavalent succimer
V09IA03
Forms & Strengths
- Kit for the preparation of technetium Tc 99m pentavalent succimer injection
- Lyophilized powder containing 2.3 mg meso-2,3-diaminosuccinic acid (DMSA)
Adult Dosing
- Diagnostic imaging: 370 to 740 MBq (10 to 20 mCi) administered via intravenous injection
- Imaging typically performed 2 to 4 hours post-injection
Pediatric Dosing
- Diagnostic imaging: Dosages adjusted based on body weight or surface area according to pediatric dosing guidelines
- Minimum administered activity generally determined by gamma camera sensitivity and institutional protocol
Indications
- Scintigraphic imaging of medullary thyroid carcinoma
- Localization of neuroendocrine tumors
- Imaging of soft tissue tumors and skeletal lesions exhibiting pentavalent DMSA uptake
Mechanism of Action
- Radiopharmaceutical agent utilizing technetium Tc 99m in a pentavalent oxidation state
- Localizes in specific tumors (such as medullary thyroid carcinoma) via mechanisms involving cellular uptake and retention in malignant tissue rather than purely physiological excretion pathways
Contraindications
- Known hypersensitivity to succimer or any component of the kit
- Pregnancy, unless the potential diagnostic benefit outweighs the potential fetal risks
- Lactation (breastfeeding should be interrupted following administration)
Adverse Reactions
- Hypersensitivity reactions including rash, pruritus, and urticaria
- Nausea and dizziness (rare)
- Injection site reactions or pain
Drug Interactions
- Chelating agents and heavy metals may alter the biodistribution or uptake of pentavalent DMSA
- Prior administration of contrast media or other radiopharmaceuticals may interfere with imaging quality
