technetium (99mTc) depreotide
V09IA05
Forms & Strengths
- Lyophilized kit (10-15 µg depreotide) for reconstitution with 99mTc pertechnetate
- Radiochemical purity ≥95% after preparation
- Prepared activity per vial: 20-30 mCi (740-1110 MBq) of 99mTc‑depreotide
Adult Dosing
- 20–30 mCi (740–1110 MBq) IV bolus
- Inject 30–60 min prior to SPECT acquisition
- Maximum single dose: 30 mCi (1110 MBq) to limit radiation exposure
Pediatric Dosing
- 0.5–1.0 mCi/kg (18.5–37 MBq/kg) IV bolus
- Minimum total activity 5 mCi (185 MBq) for adequate imaging
- Administer 30–60 min before imaging, adjusting for weight
Indications
- Evaluation of solitary pulmonary nodules (differentiating benign vs malignant)
- SPECT imaging of somatostatin‑receptor positive thoracic lesions
- Staging and detection of neuroendocrine tumors
- Localization of head and neck neoplasms expressing somatostatin receptors
Mechanism of Action
- Depreotide is a somatostatin analogue with high affinity for sst2 and sst5 receptors
- Tc‑99m label emits 140 keV gamma photons detectable by SPECT
- Binding to receptor‑rich tumor cells concentrates radioactivity for imaging
Contraindications
- Known hypersensitivity to depreotide, tin compounds, or any kit excipient
- Pregnancy (radiation risk to fetus)
- Breast‑feeding (excretion in milk)
Adverse Reactions
- Mild nausea or vomiting
- Headache or dizziness
- Transient flushing or warmth at injection site
- Rare allergic reactions: urticaria, bronchospasm, anaphylaxis
Drug Interactions
- Concurrent somatostatin analogues (e.g., octreotide) may compete for receptor binding
- Drugs altering renal clearance (e.g., diuretics) can modify biodistribution
- Cumulative radiation dose considerations with other radiopharmaceuticals
