lutetium (177Lu) vipivotide tetraxetan
V10XX05
Forms & Strengths
- Solution for intravenous infusion: 1,000 MBq/mL (1 GBq/mL) at the time of calibration
Adult Dosing
- 7.4 GBq (200 mCi) intravenously every 6 weeks for up to 6 doses
- Pre-medication with antiemetics recommended prior to administration
- Dose modification (reduction, delay, or permanent discontinuation) required for specific adverse reactions (hematologic, renal, hepatic)
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Treatment of adult patients with prostate-specific membrane antigen (PSMA)-positive metastatic castration-resistant prostate cancer (mCRPC) who have been treated with androgen receptor pathway inhibition and taxane-based chemotherapy
Mechanism of Action
- Radioligand therapeutic agent consisting of a PSMA-binding ligand coupled to the beta-minus emitting radioisotope lutetium-177
- Binds to PSMA-expressing cells (including prostate cancer cells), delivering beta-particle radiation to the target cells and surrounding microenvironment to induce DNA damage and cell death
Adverse Reactions
- Myelosuppression (anemia, thrombocytopenia, leukopenia, lymphopenia)
- Fatigue
- Nausea
- Dry mouth
- Vomiting
- Constipation
- Acute kidney injury
- Infusion-related reactions
Drug Interactions
- Concomitant use with strong CYP inhibitors or inducers has not been formally studied but pharmacokinetic interactions are unlikely given elimination via the renal system
- Avoid concurrent use of nephrotoxic drugs to minimize additive renal toxicity
